We are looking for a Community Care Officer to work 28 hours per week; with us at Lincoln County Hospital Social Work Department. You will undertake assessments of Individual Need with adults and their carers. We are developing this service with a focus on providing people with the maximum opportunity possible to regain and maintain independence to continue to live in their homes. We need an imaginative and flexible Community Care Officer to contribute to our service.
