An opportunity has arisen for a position as Clinical Case Manager / Community Matron for 33.75 hours per week in the Ellesmere Port/Neston area to join the existing team which currently comprises of both nurses and allied health professionals. We are looking for a candidate who will have either completed or be working towards a Postgraduate Certificate in Clinical Case Management and may already be an Independent Non-Medical Prescriber. Experience of case management in practice will also be an advantage. You will be an experienced and respected practitioner with evidence of leadership and innovation.
