The intensive home care team is a 7 day service that works from 8 a.m. to 9 p.m. We provide a crisis service and intensive home care to people over the age of 65 years; working closely with the community mental health teams of Hull, and the in-patient unit. We have recently developed the single point of access role which triages all referrals for older people across Hull. The team strives towards person centred care. We are looking to recruit a registered mental nurse; the post holder must be able to work with people with complex mental illness, in situations of crisis.
